I understand what the commitment entails. I will be a blessing. I will participate. I will be on time. I will memorize Scripture. I will receive feedback in my life. I will never be the same.

I understand that a signed waiver by this the parent/legal guardian of each student will be required before joining a Torch Team.

WAIVER & MEDICAL RELEASE
I, the undersigned, being the parent or legal guardian of the minor(s) named above, do hereby consent to the participation of these minor(s) in the following activity conducted by Antioch Community Church Norman, Antioch Youth, or Torch Team. I certify that the minor(s) listed above are physically fit and adequately prepared to participate in this event. I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that my child is injured or becomes ill. I authorize one or more of the above mentioned emergency contact to make emergency medical care decisions on behalf of the minor(s), if required by law or a health care provider, or another adult chaperone designated by the church I authorize these persons to act in my place to consent to all necessary and appropriate x-ray examinations, anesthetic, medical or surgical diagnosis or treatment, and hospital care. I understand that Antioch Community Church Norman, Antioch Youth, or Torch Team will not be responsible for medical expenses incurred solely on the basis of this authorization. I also understand that the designated adult chaperones reserve the right to restrict the minor(s) from any activity that they do not feel is within the physical capabilities of the minor(s).

PHOTO RELEASE
As the parent or legal guardian of the aformentioned minor(s) hereby grant Antioch Community Church permission to use their photograph or videos to be used in any and all church publications for display, educational and/or promotional purposes.

I waive any right to inspect or approve the finished product or the copy wherein my photograph or likeness appears. It is understood that my name may or may not appear in conjunction with the photograph or likeness. Additionally, I waive any right to royalties or other compensation arising or related to the use of my photograph or likeness.

I release and discharge the church from all claims, demands, and causes of action, which I have or may have by reason of this authorization. I have read this contract in its entirety, understand and agree to all of its terms.

I grant the Antioch Community Church permission to use their photograph or video in any and all church publications.
